Below there are 3 different sections:
Section I: Membership & Annual Dues deduction request
Section II: Jewish Learning Center deduction request
Section III: Adding Sections I & II.

Please Note:
If you are only filling out for Membership & Annual Dues or Jewish Learning Center, Section III, you just need to reenter the total amount requesting to pay for the Special Consideration. If you are requesting for both membership & annual fees and Jewish Learning Center, you will add the 2 sections up for a grand total of what you are requesting to pay.

Section I

In this section, you will be requesting how much you would like to be deducted from your membership:
A - Choose the type of Membership and/or Debt Service Fund and/or security fee
B - How much you are requesting to be deducted
C - The total amount you are requesting to pay
Please note that Young Single, Young Couple and Associate members only need to pay for the security fee.
Insert the amount you are asking to be deducted from the total amount of your membership fee.
(A) - (B) = (C)
Example:
(A) Choosing Family Membership Category, Debt Service and Security = $4,170
(B) Requesting - $500 off of total membership dues

(C) Write in the box above: $3,670
Section II

In this section, you will be requesting how much you would like to be deducted from your Jewish Learning Center Tuition:
A - Adding in the number of students attending JLC & which grades
B - How much you are requesting to be deducted from JLC tuition
C - The total amount you are requesting to pay for JLC Tuition
   Kindergarten + 1st Grade ($1,269 per child)
   2nd + 3rd Grade ($1,873 per child)
   4th - 6th Grade ($1,873 per child)
Insert the amount you are asking to be deducted from the total amount of your JLC tuition.
(A) - (B) = (C)
Example:
(A) 1 Child in 1st Grade ($1,400) + 1 Child in 3rd Grade ($2,050) = $3,450
(B) Requesting - $1000 off of total tuition

(C) Write in the box above: $2,450
Section III

Add Section I (C) & Section II (C) together to get the GRAND TOTAL.
Please add to TOTAL cost you are requesting to pay. If you are only requesting a deduction for membership or Jewish Learning Center, just enter in the number from part C of that section.

* Subject to approval of your request. Please note that we have a certain amount of funds allocated to Special Considerations, please take that into consideration when you are requesting financial help. The funds will be distributed on a first-come-first-serve basis. Once the funds have been exhausted, we will no longer be able to accommodate any more requests. Thank you.
